New Patients.

 

We welcome new patients to the practice (even on the NHS) but please note that we are very busy and rarely have an appointment available immediately.

 

All new patients need to come into the practice personally to have their details entered into the system before we can offer an appointment.

Please fill these forms before you come to be entered on our system-

  1. New patient Registration

  2. Medical History Form

 

**** PLEASE NOTE THERE IS A STRICT POLICY IF YOU FAIL TO ATTEND ANY APPOINTMENTS/ OR CANCEL AT SHORT NOTICE. If this happens you WILL be blocked from the practice. Please don't send an appeal asking to be reconsidered*****

New Registrations please note:

  • We are currently only registering New Patients on:

    • Tuesday from 1000am to 1200pm and 230pm to 400pm

    • Thursday from 1000am to 1200pm and 230pm to 400pm

  • We are limiting how many patients per day we can accept.

  • Due to our workload- There is no guarantee you will get registered if you turn up.

 

All new patients need to bring the following paperwork with them:

 

  1. Proof of I.D (eg. Photocard driving license or Original passport) **

  2. New Patient Registration form filled and signed- (downloaded from here)**

  3. Medical history form filled out with all the details

  4. Proof of address: (eg. Recent Utility bill or Credit Card Bill or Social Security paperwork) **

  5. Registered Doctors Surgery address and Telephone number.

  6. NHS Number (This can be obtained from your Doctor)

  7. National Insurance Number (if applicable)

 

      ** These are mandatory

Please call reception on 020 8573 3048 or 020 8573 9299 to make an appointment where our reception team will be more than happy to help you.

If you have received a recall letter reminding you to book your check up then please keep your Reference number handy.

Emergency Appointments:

During Normal Working hours:

If you are NOT REGISTERED with the practice and need an URGENT/ EMERGENCY appointment then please call reception for a price and availability of an appointment.

All registered patients will need to call reception to book an emergency appointment. These will be offered on an availability basis and the most urgent patients will be given priority.

Outside Normal Working hours:

If you have a dental emergency when the surgery is closed, during evenings and weekends,

please Call 111 where you will be assessed and booked in to see an emergency dentist, if necessary.

This emergency service operates:

6pm to 8am- Monday to Friday

24 hours/day - Weekends & Bank Holidays
